• Young people can self-refer (you can ask a friend or family member to help you complete the form - but they should have your permission)

• Those working with/on behalf of a young person

How do I submit a referral?

• Download the referral form below

• Attach it to an email and send it to kim@bardsleyyouth.org

• Put "New Referral" as your email subject
